- This agreement may be terminated at any time providing one month’s notice to The Dentist or MembershipPlans Ltd t/a Privilege Plan.
- Your Dentist may terminate this agreement by providing one month’s notice in writing.
- If the Direct Debit is not paid, the Dentist or Membership Plans Ltd t/a Privilege Plan may terminate this agreement by giving The Patient one month’s notice in writing.
- The monthly fee paid by Direct Debit may be increased by The Dentist providing one month’s notice in writing to The Patient.
- The Patient will pay the fee payable by Direct Debit to Membership Plans Ltd t/a Privilege Plan, who collect fees on behalf of The Dentist.
- The Patient acknowledges that Membership Plans Ltd t/a Privilege Plan are administrators and a collection agent on behalf of The Dentist and as such are not responsible for standards of dental treatment, service or care provided by The Dentist.
- The Patient is responsible for attending the practice on a regular basis.
- This agreement is not transferable between The Patient and other patients and only covers the services available with The Dentist and his/her nominated deputy in his/her absence.
- The Dentist, or, his or her nominated deputy will provide dental services as described in the practice information leaflet.
- In the event of the sale of the practice to a new practice owner then this agreement can be transferred to the new practice owner.
- For details of additional benefits provided, please check with your Dentist or refer to the patient information leaflet provided.
- Entitlements not used in any given period may not be carried forward.
- The Patient agrees to pay fees to the Dentist for additional dental treatment completed that is not included in this plan.
- The Patient should notify Membership Plans Ltd t/a Privilege Plan of the change of any details as provided on this form, i.e. address, telephone number.
- Any failure to attend an appointment covered by this agreement may result in a fee being charged at the next appointment.
- Patients are responsible for making and attending appointments provided within the plan, no refunds will be provided.
- Your membership benefits will commence following your first monthly payment.
